2023 Heart Failure Awareness Week:

HF Refresh Guidelines Update


Heart Failure (HF) remains a growing concern in healthcare, with nearly 6.5 million Americans currently living with heart failure and approximately 550,000 new cases diagnosed each year. Heart failure contributes to approximately 287,000 deaths per year and carries a 50% mortality rate within 5 years of diagnosis. Deaths from heart failure have decreased on average by 12% per decade for women and men over the past fifty years- attributable to ongoing research and revision of care standards and medical guidelines. Even so, patients with heart failure are still well known for their risk for serious complications related to their disease process, as well as unintended consequences of medications used to manage their condition.

The Heart Failure Awareness Campaign for 2023 is themed “HF Refresh” to ensure the most current guideline directed heart failure education reaches the hands of our patients and their families. The goal is to provide optimal resources for patients, health care providers, and families to improve the quality of life for our patients. Guideline directed medical care has ensured we provide top quality medical care to improve the outcomes for our heart failure patients. Our main areas of focus are listed below:

Sometimes heart failure patients feel alone. Uncertain and afraid, they worry about the details of their self-care, sometimes without a solid understanding of their situation. The moderated community site offers support and an avenue for listening ears and advice about living with heart failure. Nurses and providers are welcome to join too!

 

The American heart Association is hosting an educational webinar that introduces the Heart Failure System of Care including the challenges patients and providers face in delivering the recommended guideline directed care.
