Documentation - Download the CHFN Tracker form (located on the dropdown menu of Recertification tab). Complete with CEs and Professional Development. Do not submit certificates in lieu of completing the form. These are not acceptable and will delay the evaluation of your application.

Do NOT upload certificates or other documentation.

Within 2-4 weeks of receiving your application, AAHFN will confirm if your application has been accepted. You are advised to keep a copy of your completed application for your records. AAHFN is not responsible for correspondence lost in the mail.

The name on your AAHFN profile will be the name on your certificate. In addition, the address on your AAHFN profile, is the address that your certificate will be mailed to. It is your responsibility to update your profile. There is a $10 fee if certificates need to be re-mailed.
