Dyspnea Checklist

Based upon feedback from a number of HF physicians and nurses, this checklist had been developed as a tool to assist clinicians with the evaluation of dyspnea during hospitalization. Research has shown that many patients are discharged too quickly, before their congestion is adequately treated, leading to increased morbidity and mortality. One reason for this may be that assessing dyspnea is quite challenging, especially during hospitalization. The Dyspnea Checklist, when utilized on a daily or twice a day basis, provides a scoring system for objective evaluation and serial measurement of changes in dyspnea.
