Purpose Statement

The purpose of this activity is to improve nursing practice for heart failure home care patients through enriched knowledge of chronic care management regarding diet, physical activity, and medication reconciliation.

 

Learner Outcomes

  1. The nurse will discuss strategies to prevent malnutrition for at risk HF patients in the home care setting.
  2. The nurse will recognize daily sodium intake recommendations for HF home care patients.
  3. The nurse will examine the medication reconciliation process for HF patients discharged home.
  4. The nurse will distinguish physical therapy indicators for HF home care patients.
