AAHFN-CB Recertification Audit Frequently Asked Questions

 

Randomly, the AAHFN-CB will select and audit recertification applicants. If your application is chosen for audit, you will be asked to submit supporting documentation. You will be contacted by email with instructions on how to submit audit documentation.
Certificants will have 60 days from the date of the email letter to submit the required verification materials.

If materials are not submitted to AAHFN-CB within 60 days, certification will be revoked and notices may be sent to the candidate's employer and state Board of Nursing, where appropriate.

I recertified using the CE option. What documentation do I need to send AAHFN-CB?
You must provide a copy of continuing education (CE) certificate/s for only the contact hours submitted for recertification. Documentation submitted must include the date, number of contact hours earned, title of course and name of the accrediting agency. If the words 'heart failure' are not present in the course's title or the organization/agency name that held the course(s) and the topic is NOT heart failure specific, we will need to see a syllabus outline/agenda of course content to be able to give you contact hour credit. The audit process is separate from the recertification process.

What if my CE certificate does not list an accrediting agency?
Sometimes the accrediting agency is shown in smaller print on the certificate. If you cannot find an accrediting agency listed on your certificate, please contact the issuer of your certificate for that information. AAHFN-CB will need written documentation from the accrediting agency that contact hours were awarded for the CE offering.

What if I can't find my CE certificates?
You are responsible for keeping a copy of your certificates. If you can't find your AAHFN CE certificates from annual meetings, symposia, or online CE, there will be a $50 retrieval fee for AAHFN staff to send you a copy.

What documentation will I need to submit for the recertification option of academic credit?
Provide a copy of the official transcript documenting completion of semester or quarter hours of academic credit. We must be able to see your name, semester and year, course title, course credits possible/earned and official grade. Academic courses in progress are not acceptable.

What documentation do I need to submit for the recertification option of my lecture presentations?
Provide (1) a PDF of the official brochure with your lecture title, name, etc. listed and sponsoring organization or (2) an acceptance letter/notice of the oral presentation by the sponsoring organization (if you did not attend the full meeting and receive CE credit). If you do not have evidence of # 1 or 2 above, provide the lecture outline, name of sponsoring organization and documentation of awarded CEs. What documentation do I need to submit for the recertification option of my scholarly posters?
Provide documentation demonstrating the poster content (for example, a PDF of the poster content/layout) and acceptance letter/electronic mail notice of the poster by the sponsoring organization.

What documentation do I need to submit for the recertification option of authorship?
For journal articles, provide proof of authorship by submitting a copy of the title page of the paper that includes the paper title, your name as author, journal name and publication information (issue, volume, pages and date). For book authorship, submit page(s) from the book that identify the book and/or chapter title, your authorship and publication information, including year of publication.

What documentation do I need to submit for the recertification option of clinical hours?
A portion of your audit letter will include a form on which you will document practice hours earned, the name of the clinical setting, and the time period practiced. This form must be signed by a supervisor to verify your clinical practice reported. We will contact your supervisor for verification. If the person's name you supplied is not your current supervisor (due to job change or retirement) when we conduct the audit, we will need you to supply your current supervisor's name and contact information so we can verify your hours. Please do NOT supply a colleague's name in lieu of a supervisor. Only a supervisor can verify clinical hours completion.

What documentation do I need to submit for the recertification option of development of a program, service, or tool used for in the management of patients with heart failure?
To verify requirements, you will need to (1) provide the finished document (program or tool) if applicable, (2) briefly describe the program, service or tool and how it is used, (3) provide explicit information about your role in the development process (copies of official meeting minutes with you listed in attendance and your role(s) can suffice), and (4) provide a brief timeline that includes the date the development started and date of implementation. If development involved a group, provide your role and the role of other group members. If you were not the leader, please list the group leader’s name and job title. Finally, provide the amount of hours you estimate that you personally provided to complete the development process of the program, service or tool.

What qualifies as a professional society? 

Professional society means AAHFN, ACC, AHA. Cannot be hospital based leadership role or a local community group - must be a professional organization that is incorporated/legally recognized.) Must be a national forum. Volunteering for AAHFN in a leadership role counts, such as chair of a committee.

What documentation do I need to submit for recertification option served in a leadership role with a professional society?
To verify your leadership role within a professional society, provide the official roster for each year of service in the specific leadership role. The roster should be on official organization stationary (with masthead). If no roster is available or the official roster does not include the organization’s masthead or year(s) of service, please request a letter of verification of your role from the professional society and request that it include the year of volunteerism, and specific leadership role. Of note, professional societies must be healthcare related.

What documentation do I need to submit for recertification option for participating in a quality improvement project related to heart failure care?
