Purpose Statement
The purpose of this activity is to enable the learner to apply adult learning principles and teaching strategies to the education of the adult patient with heart failure.

Objectives
1. Identify common problems with medication reconciliation
2. Discuss techniques to improve quality of medication reconciliation activities in different practice settings
3. Recognize key components of health behavior change
4. Utilize the core principles of motivational interviewing within one's clinical practice
5. Utilize the teach-back method within one's clinical practice
6. Identify the key parts of the education requirements for patients with HF as mandated by AHA/ACC, HFSA, and CMS
7. Recognize the impact of cognitive impairment on the ability of the patient to understand instructions


Requirement for successful completion

Read the issue in its entirety and the accreditation information below; the articles that have the education logo next to it are part of the enduring educational activity Heart Failure Patient Education. Access the post test, successfully pass it with a minimal score of 70% and submit an evaluation and receive a certificate of completion. Participants who successfully complete this activity in its entirety will be awarded .75 contact hour.
