PURPOSE
The purpose of this activity is to introduce the reader to team members involved in the care of heart failure patients and enable the learner to understand the value in providing a team-based approach in the management of this patient population.

OBJECTIVES
1. Discuss basic telehealth technologies and their application to the healthcare model and associate the impact of nursing knowledge of telehealth on the changing healthcare model.
2. Describe evidence in use of telehealth in the heart failure population to improve patient care outcomes and overall benefits to the patient.
3. Creatively apply telehealth to heart failure patient management in their own patient care setting.


REQUIREMENTS FOR SUCCESSFUL COMPLETION
Participant must read the activity, the accreditation information, complete the activity in its entirety, successfully pass the post test with a minimal score of 70% and submit an evaluation to be awarded contact hours. Participants who successfully complete this activity in its entirety will be awarded .75 contact hours.
