PURPOSE
The purpose of this activity is to identify factors that affect/predict higher readmission rates and what strategies have been effective at reducing 30-day readmission rates for patients admitted with HF to improve patient outcomes and mitigate the cost to hospitals for unreimbursed care.

OBJECTIVES
1.Recognize the significance of 30 day readmission rates for heart failure to patients and healthcare institutions. 
2. Discuss the factors leading to risk for higher readmission rate including clinical parameters, laboratory markers, and psychosocial factors.
3. Identify 3 evidence-based strategies for lowering the 30 day readmission rate. 


REQUIREMENTS FOR SUCCESSFUL COMPLETION
Participant must read the accreditation information, complete the activity in its entirety, successfully pass the post test with a minimal score of 70% and submit an evaluation to be awarded contact hours.
Participants who successfully complete this activity in its entirety will be awarded 1.3 contact hour.
