PURPOSE
The purpose of this activity is to introduce the reader to team members involved in the care of heart failure patients and enable the learner to understand the value in providing a team-based approach in the management of this patient population.

OBJECTIVES
1. Define the incidence and prevalence of HF and the associated economic and psychosocial.
2. Discuss how inpatient heart failure teams measure success and team members that should be included.
3. Identify systems of care that should be implemented to promote care coordination for patients with chronic HF.
4. Discuss transitional care interventions in the care of heart failure patients.


REQUIREMENTS FOR SUCCESSFUL COMPLETION
Participant must read the activity, the accreditation information, complete the activity in its entirety, successfully pass the post test with a minimal score of 70% and submit an evaluation to be awarded contact hours. Participants who successfully complete this activity in its entirety will be awarded 1 contact hours.
